Verify the patient is feeling well at appointment confirmation and check-in. Below are the questions to be answered. If there are any concerns the appointment should be rescheduled.

Confirmation calls:

· Are you experiencing a fever, cough, shortness of breath or difficulty breathing?

· Have you had any of these symptoms in the last 14 days?

· In the past 15 days, have you traveled internationally, or come into close contact with anyone who has?

· Have you been in close contact with someone who tested positive for coronavirus (COVID-19) in the last 14 days?

Check in:

· Check for fever over 100 degrees
